We are so excited to announce that the office is certified with MassHealth to assist with MassHealth applications.
If you need assistance please call the office and we can schedule you a time to come in to process the application.
Documentation required are as follows:
- Proof of ID for all adults applying.
- Social security card/proof of application for SSN# for everyone applying.
- Pay stubs for all adults applying, 4 pay stubs if paid weekly, 2 if paid biweekly.
- Proof of any income.
- In some occasions birth certificate for the children is required.
- Proof of residency/address for all adults.
- Proof of current legal status for everyone in the application.
Prepare For Your Visit
Co-payments will be collected at the time of service.
Know the benefits and limitations of your insurance plan. Talk with your insurance carrier and verify that we are in-network providers with your plan.
Bring your insurance card with you to each appointment.
We will help you enroll or switch your insurance plan to one that we take, including Tufts & MassHealth. For contracted insurances plans, we will submit claims, otherwise, payment is expected at the time of your appointment.
Payment for non-covered services is required at the time of service. We accept checks, Visa, Mastercard, Discover, and American Express.
